This is due to operation of the watch which uses a sensor to detect contact with the skin. This saves the user, as long as the watch is worn, repeat entering their PIN.
This problem has also been confirmed by Apple. They said, "the permanent or temporary changes to your skin, such as some tattoos, can also affect the performance of the heart rate sensor. The ink, pattern and saturation of some tattoos can block the light sensor, which makes obtaining reliable measurements difficult. " However, it depends on the tattoo. So, you may still want to give it a shot anyway.
